Public Health Ethics Forum Evaluation

April is National Minority Health Month in the United States. The National Center for Bioethics in Research and Health Care at Tuskegee University, CDC Office of Minority Health and Health Equity invite you to the 2019 Public Health Ethics Forum on April 26, 2019, from 8:00 a.m. to 4:15 p.m. EDT at the Tom Harkin Global Communications Center, CDC-Atlanta.

The year’s theme is Ethical Dilemmas in Child and Adolescent Health. Please register for in person attendance or virtual participation.

Instructions for non-CDC staff attending the Public Health Ethics Forum in person:  

Please contact Kayla Johnson at (404) 639-3075 or e-mail: ood5@cdc.govto submit your name for security clearance or with any questions regarding the security clearance process.

All non-CDC staff will be required to complete a security clearance request form. The processing time forU.S. citizens is 3 business days and non-US citizens is 10 full business days. All visitors must bring government-issued photo identification with them to the CDC campus. Please have oneof the following: 1) Valid U.S. driver’s license, 2) Valid Passport, or 3) Valid state issued photo identification card.

Lunch information:

Lunch for the 2019 Public Health Ethics Forumis on your own, however, an option to order lunch from Which Wich restaurant is available. Please go online to https://whichwich.olo.express/menu/which-wich-emory-point. The restaurant location that will be preparing the meal is Emory Point in Atlanta. Order your lunch online and prepay via the online Which Wich website. When paying online for your lunch from Which Wich, you must tag your lunch, 2019PHEF CDC. Your boxed lunch and beverage will be delivered on Friday, April 26. If you have difficulty ordering online, please call Which Wich at 404-549-8889 and purchase your meal.

When you order lunch on the Which Wich website, please ensure you adhere to the following instructions.

  • You can only order Catering Box Lunches
  • Add meeting name, “2019 PHEF CDC”in the Special Instructions field
  • You can add a drink to your boxed lunch by clicking the beverages tab, selecting a drink of choice and adding it to the cart
  • Select“Pick Up” even though the order will be delivered, this will make sure that you aren’t charged a delivery fee
  • Select the “Later” button and add April 26, 11 a.m. (Do not use “Now”)
  • The cut-off to order lunch is by 5 p.m. on April 25
